Due to the recent passing of the Employer Reporting Improvement Act, Sigvaris has altered its distribution method for 1095-C tax forms beginning January 2025.  (The 1095-C tax form documents an employee’s eligibility for and acceptance or declination of coverage through the employer’s group health plan.)

Sigvaris will no longer automatically distribute these to all current and former employees for the tax year. 

The forms, are, however, still available by requesting one from the Human Resources department by contacting:  jennifer.kidd@sigvaris.com or Kristi.smith@sigvaris.com.
